Kontrata u n\u00ebnshkrua me kompanin\u00eb \u201cSaniService sh.p.k.\u201d p\u00ebr nj\u00eb periudh\u00eb 10-vje\u00e7are, duke hyr\u00eb n\u00eb fuqi n\u00eb dhjetor t\u00eb po atij viti.<\/p>\n\n\n\n<p>Objektivi i k\u00ebtij koncesioni ishte p\u00ebrmir\u00ebsimi i cil\u00ebsis\u00eb dhe siguris\u00eb s\u00eb sh\u00ebrbimeve kirurgjikale n\u00eb spitalet publike n\u00eb t\u00eb gjith\u00eb vendin. Por me kalimin e viteve, ky projekt u kthye nga nj\u00eb premtim modernizimi n\u00eb nj\u00eb shembull flagrant t\u00eb sfidave q\u00eb shoq\u00ebrojn\u00eb kontratat koncesionare n\u00eb Shqip\u00ebri.<\/p>\n\n\n\n<p>Nd\u00ebrsa ideja e financimit \u201cpay-per-use\u201d, ku shteti paguan p\u00ebr \u00e7do p\u00ebrdorim t\u00eb sh\u00ebrbimit, synonte t\u00eb sillte eficienc\u00eb dhe kontroll t\u00eb kostove, realiteti rezultoi krejt ndryshe. Nga viti 2017 deri n\u00eb 2021, pagesat ndaj kompanis\u00eb kap\u00ebn shifr\u00ebn e 7.88 miliard\u00eb lek\u00ebve, nd\u00ebrsa n\u00eb total, buxheti parashikon t\u00eb paguaj\u00eb deri n\u00eb fund t\u00eb kontrat\u00ebs rreth 16.5 miliard\u00eb lek\u00eb.<\/p>\n\n\n\n<p>N\u00eb m\u00ebnyr\u00eb t\u00eb befasishme, vet\u00ebm p\u00ebr vitin 2018, shpenzimet tejkaluan ndjesh\u00ebm parashikimet fillestare: nga 1.26 miliard\u00eb lek\u00eb t\u00eb planifikuara, u faturuan rreth 2.08 miliard\u00eb lek\u00eb. Nj\u00eb rritje e till\u00eb nuk u shoq\u00ebrua me asnj\u00eb analiz\u00eb t\u00eb thelluar apo nd\u00ebrhyrje p\u00ebr t\u00eb mbajtur kostot n\u00ebn kontroll.<\/p>\n\n\n\n<p>P\u00ebr m\u00eb tep\u00ebr, strukturat p\u00ebrgjegj\u00ebse nuk kishin parashikuar mekanizma p\u00ebr monitorim dhe kontroll t\u00eb zbatimit t\u00eb kontrat\u00ebs n\u00eb terren. N\u00eb auditimet e kryera nga Kontrolli i Lart\u00eb i Shtetit (KLSH), evidentohet se mungon nj\u00eb nj\u00ebsi funksionale p\u00ebr kontrollin fizik t\u00eb k\u00ebtij sh\u00ebrbimi, nj\u00eb neglizhenc\u00eb serioze p\u00ebr nj\u00eb kontrat\u00eb q\u00eb r\u00ebndon ndjesh\u00ebm mbi financat publike.<\/p>\n\n\n\n<p><strong>Standarde jo t\u00eb barabarta, sete t\u00eb pap\u00ebrshtatshme<\/strong><\/p>\n\n\n\n<p>Problemet e koncesionit t\u00eb sterilizimit nuk kufizohen vet\u00ebm n\u00eb tejkalimet buxhetore dhe munges\u00ebn e monitorimit institucional. Nj\u00eb nd\u00ebr \u00e7\u00ebshtjet m\u00eb shqet\u00ebsuese q\u00eb kan\u00eb dal\u00eb nga auditimet e Kontrollit t\u00eb Lart\u00eb t\u00eb Shtetit (KLSH) ka t\u00eb b\u00ebj\u00eb me munges\u00ebn e standardizimit dhe koherenc\u00ebs n\u00eb ofrimin e seteve kirurgjikale.<\/p>\n\n\n\n<p>T\u00eb dh\u00ebnat e vitit 2022 tregojn\u00eb nj\u00eb praktik\u00eb t\u00eb paqart\u00eb dhe t\u00eb pasistemuar n\u00eb shp\u00ebrndarjen e seteve kirurgjikale n\u00eb spitale t\u00eb ndryshme. P\u00ebr t\u00eb nj\u00ebjt\u00ebn nd\u00ebrhyrje kirurgjikale, pra p\u00ebr t\u00eb nj\u00ebjtin lloj operacioni me t\u00eb nj\u00ebjtat k\u00ebrkesa klinike,&nbsp; jan\u00eb p\u00ebrdorur sete t\u00eb ndryshme instrumentesh. Kjo jo vet\u00ebm q\u00eb cenon standardet klinike dhe sigurin\u00eb e pacientit, por krijon edhe nj\u00eb konfuzion t\u00eb madh n\u00eb logjistik\u00ebn e ofrimit t\u00eb sh\u00ebrbimit dhe n\u00eb menaxhimin e kostove.<\/p>\n\n\n\n<p>N\u00eb praktik\u00eb, mungesa e nj\u00eb protokolli t\u00eb unifikuar p\u00ebr p\u00ebrcaktimin e p\u00ebrmbajtjes s\u00eb seteve t\u00eb personalizuara sjell pasoja t\u00eb shumta. Nga nj\u00ebra an\u00eb, stafi kirurgjikal nuk ka garanci p\u00ebr pajisje t\u00eb nj\u00ebjta dhe t\u00eb p\u00ebrshtatshme nga nj\u00eb nd\u00ebrhyrje n\u00eb tjetr\u00ebn. Kjo mund t\u00eb ndikoj\u00eb drejtp\u00ebrdrejt n\u00eb koh\u00ebzgjatjen e nd\u00ebrhyrjes, n\u00eb p\u00ebrgatitjen para operacionit dhe potencialisht n\u00eb sigurin\u00eb intra-operatore t\u00eb pacientit. Nga ana tjet\u00ebr, p\u00ebr operatorin koncesionar, kjo lejon hap\u00ebsir\u00eb p\u00ebr interpretim subjektiv t\u00eb kompleksitetit t\u00eb nd\u00ebrhyrjes, dhe p\u00ebr rrjedhoj\u00eb, p\u00ebr faturim t\u00eb lart\u00eb t\u00eb seteve \u201cm\u00eb komplekse\u201d, pa nj\u00eb justifikim t\u00eb qart\u00eb mjek\u00ebsor.<\/p>\n\n\n\n<p>Raporti i KLSH-s\u00eb evidenton se ka pasur nj\u00eb <strong>rritje jo proporcionale t\u00eb p\u00ebrdorimit t\u00eb seteve t\u00eb kompleksitetit t\u00eb lart\u00eb<\/strong><strong>,<\/strong> q\u00eb p\u00ebrkthehet n\u00eb nj\u00eb rritje artificiale t\u00eb \u00e7mimeve t\u00eb sh\u00ebrbimit, duke nxitur p\u00ebrdorimin e seteve m\u00eb t\u00eb shtrenjta edhe n\u00eb nd\u00ebrhyrje q\u00eb nuk e k\u00ebrkojn\u00eb k\u00ebt\u00eb nivel kompleksiteti. Edhe kategorit\u00eb e seteve me kompleksitet t\u00eb ul\u00ebt jan\u00eb rritur me 16%, nd\u00ebrsa ato me kompleksitet t\u00eb mes\u00ebm me 3%, duke d\u00ebshmuar p\u00ebr nj\u00eb munges\u00eb t\u00eb nj\u00eb matrice t\u00eb qart\u00eb p\u00ebr kategorizimin e nd\u00ebrhyrjeve dhe p\u00ebr p\u00ebrcaktimin transparent t\u00eb \u00e7mimeve p\u00ebr secil\u00ebn kategori.<\/p>\n\n\n\n<p>P\u00ebr m\u00eb tep\u00ebr, n\u00eb auditimin e kryer n\u00eb spitale universitare, rajonale dhe bashkiake \u00ebsht\u00eb konstatuar se p\u00ebr t\u00eb nj\u00ebjt\u00ebn diagnoz\u00eb kirurgjikale jan\u00eb p\u00ebrdorur deri n\u00eb 3\u20134 lloje t\u00eb ndryshme setesh, gj\u00eb q\u00eb cenon r\u00ebnd\u00eb parimin e <strong>barazis\u00eb s\u00eb aksesit n\u00eb sh\u00ebrbim<\/strong> p\u00ebr qytetar\u00ebt n\u00eb zona t\u00eb ndryshme t\u00eb vendit. Nj\u00eb pacient n\u00eb Tiran\u00eb mund t\u00eb ket\u00eb marr\u00eb nj\u00eb set m\u00eb t\u00eb kompletuar ose m\u00eb t\u00eb shtrenjt\u00eb, nd\u00ebrsa nj\u00eb pacient n\u00eb nj\u00eb spital rajonal mund t\u00eb ket\u00eb p\u00ebrfituar nj\u00eb set m\u00eb t\u00eb thjesht\u00eb p\u00ebr t\u00eb nj\u00ebjt\u00ebn nd\u00ebrhyrje, duke krijuar k\u00ebshtu nj\u00eb ndarje t\u00eb pabarabart\u00eb t\u00eb cil\u00ebsis\u00eb s\u00eb kujdesit.<\/p>\n\n\n\n<p>Kjo munges\u00eb uniformiteti ngre dyshime serioze edhe p\u00ebr <strong>munges\u00ebn e mbik\u00ebqyrjes teknike dhe klinike nga ana e autoriteteve shtet\u00ebrore<\/strong>, t\u00eb cilat duhet t\u00eb ishin garantues t\u00eb zbatimit t\u00eb protokolleve dhe kontrollit t\u00eb cil\u00ebsis\u00eb s\u00eb pajisjeve kirurgjikale n\u00eb t\u00eb gjith\u00eb rrjetin spitalor.<\/p>\n\n\n\n<p>N\u00eb fund, kjo situat\u00eb nuk \u00ebsht\u00eb thjesht nj\u00eb problem i teknikave administrative apo buxhetore \u2013 por nj\u00eb \u00e7\u00ebshtje e r\u00ebnd\u00eb sistemike q\u00eb cenon <strong>besimin publik<\/strong><strong>,<\/strong> krijon <strong>shpenzime t\u00eb pajustifikuara<\/strong> p\u00ebr taksapaguesit dhe ekspozon pacient\u00ebt ndaj <strong>rrezikut t\u00eb nj\u00eb kujdesi jo uniform dhe potencialisht t\u00eb pasigurt<\/strong><strong>.<\/strong><\/p>\n\n\n\n<p>P\u00ebrball\u00eb k\u00ebsaj panorame, nevoja p\u00ebr nj\u00eb <strong>rishikim t\u00eb thelluar t\u00eb struktur\u00ebs s\u00eb kontrat\u00ebs, protokolleve t\u00eb seteve kirurgjikale dhe transparenc\u00ebs n\u00eb faturim<\/strong> \u00ebsht\u00eb urgjente dhe thelb\u00ebsore p\u00ebr t\u00eb rikthyer standardet n\u00eb vendin q\u00eb u takon n\u00eb sistemin sh\u00ebndet\u00ebsor publik.<\/p>\n\n\n\n<p><strong>Reagimi institucional dhe hetimet p\u00ebr abuzime<\/strong><\/p>\n\n\n\n<p>N\u00eb vitin 2018, KLSH i k\u00ebrkoi Ministris\u00eb s\u00eb Financave dhe Ministris\u00eb s\u00eb Sh\u00ebndet\u00ebsis\u00eb t\u00eb rishikonin struktur\u00ebn e \u00e7mimeve dhe paketave q\u00eb ofronte koncesionari. U rekomandua q\u00eb t\u00eb p\u00ebrjashtoheshin nga p\u00ebrfshirja n\u00eb kosto element\u00eb q\u00eb mbulohen tashm\u00eb nga spitalet, si\u00e7 jan\u00eb energjia apo uji. Po ashtu, KLSH rekomandoi riformatimin e grupeve t\u00eb kompleksitetit dhe p\u00ebrshtatjen e seteve sipas diagnoz\u00ebs dhe tipit t\u00eb nd\u00ebrhyrjes. Por deri m\u00eb sot nuk ka prova t\u00eb qarta q\u00eb k\u00ebto rekomandime jan\u00eb zbatuar.<\/p>\n\n\n\n<p>Koncesioni i sterilizimit nuk ka mbetur jasht\u00eb radarit t\u00eb Prokuroris\u00eb s\u00eb Posa\u00e7me (SPAK). Q\u00eb nga viti 2020 jan\u00eb n\u00eb zhvillim hetime q\u00eb p\u00ebrfshijn\u00eb ish-zyrtar\u00eb t\u00eb nivelit t\u00eb lart\u00eb n\u00eb Ministrin\u00eb e Sh\u00ebndet\u00ebsis\u00eb, p\u00ebrfshir\u00eb ish-ministrin Ilir Beqaj, ish-zv\/ministrin Klodian Rrjepaj dhe p\u00ebrfaq\u00ebsues t\u00eb kompanis\u00eb \u201cSaniService\u201d. Akuza t\u00eb r\u00ebnda si shp\u00ebrdorim detyre dhe shkelje t\u00eb barazis\u00eb n\u00eb tendera jan\u00eb b\u00ebr\u00eb publike, duke i dh\u00ebn\u00eb nj\u00eb tjet\u00ebr dimension kritik k\u00ebtij projekti.<\/p>\n\n\n\n<p>Rasti i koncesionit t\u00eb sterilizimit \u00ebsht\u00eb nj\u00eb nd\u00ebr ilustrimet m\u00eb t\u00eb qarta t\u00eb m\u00ebnyr\u00ebs se si mungesa e analiz\u00ebs s\u00eb kostove reale, mungesa e monitorimit dhe ndarja jo e balancuar e riskut mund t\u00eb kthejn\u00eb nj\u00eb projekt me q\u00ebllim pozitiv n\u00eb nj\u00eb barr\u00eb financiare p\u00ebr buxhetin publik dhe nj\u00eb burim pasigurie p\u00ebr sistemin sh\u00ebndet\u00ebsor.<\/p>\n\n\n\n<p>Kjo p\u00ebrvoj\u00eb tregon se \u00e7do projekt i ngjash\u00ebm duhet t\u00eb p\u00ebrfshij\u00eb m\u00eb shum\u00eb transparenc\u00eb, ekspertiz\u00eb t\u00eb pavarur n\u00eb analiz\u00ebn e kostove dhe p\u00ebrfitimeve, si dhe mekanizma efektiv\u00eb t\u00eb kontrollit. Nga viti 2017 deri n\u00eb 2021, pagesat ndaj kompanis\u00eb kap\u00ebn shifr\u00ebn e 7.88 miliard\u00eb lek\u00ebve, nd\u00ebrsa n\u00eb total, buxheti parashikon t\u00eb paguaj\u00eb deri n\u00eb fund t\u00eb kontrat\u00ebs rreth 16.5 miliard\u00eb lek\u00eb. N\u00eb m\u00ebnyr\u00eb t\u00eb befasishme, vet\u00ebm p\u00ebr vitin 2018, shpenzimet tejkaluan ndjesh\u00ebm parashikimet fillestare: nga 1.26 miliard\u00eb lek\u00eb t\u00eb planifikuara, u faturuan rreth 2.08 miliard\u00eb lek\u00eb. Nj\u00eb rritje e till\u00eb nuk u shoq\u00ebrua me asnj\u00eb analiz\u00eb t\u00eb thelluar apo nd\u00ebrhyrje p\u00ebr t\u00eb mbajtur kostot n\u00ebn kontroll.
