{"id":2276,"date":"2023-11-03T07:57:05","date_gmt":"2023-11-03T07:57:05","guid":{"rendered":"https:\/\/transparency.com.al\/?p=2276"},"modified":"2023-11-03T07:57:06","modified_gmt":"2023-11-03T07:57:06","slug":"klsh-kontrata-problematike-e-ppp-se-laboratorit-po-rrezikon-jeten-e-pacienteve","status":"publish","type":"post","link":"https:\/\/old.transparency.com.al\/en\/klsh-kontrata-problematike-e-ppp-se-laboratorit-po-rrezikon-jeten-e-pacienteve\/","title":{"rendered":"KLSH: Kontrata problematike e PPP s\u00eb laboratorit po rrezikon jet\u00ebn e pacient\u00ebve"},"content":{"rendered":"<p><strong>\u201cP\u00ebrcaktimi i tavaneve vjetore buxhetore po kufizon kryerjen e analizave laboratorike p\u00ebr secilin spital, t\u00eb sanksionuar n\u00eb kontrat\u00ebn koncesionare t\u00eb laborator\u00ebve, duke rrezikuar ofrimin e analizave laboratorike, n\u00eb mas\u00ebn e duhur, sipas nevojave spitalore dhe duke sjell\u00eb p\u00ebr pasoj\u00eb drejtimin e pacient\u00ebve n\u00eb marrjen e k\u00ebtij sh\u00ebrbimi tek subjektet private, me kosto p\u00ebr qytetar\u00ebt, si dhe p\u00ebrkeq\u00ebsim t\u00eb sh\u00ebndetit nga moskryerja n\u00eb koh\u00eb e analizave p\u00ebrkat\u00ebse\u201d, evidentoi Kontrolli i Lart\u00eb i Shtetit n\u00eb auditin q\u00eb ushtroi n\u00eb Fondin e Sigurimit t\u00eb detyruesh\u00ebm Sh\u00ebndet\u00ebsor p\u00ebr vitin 2022<\/strong>&#8220;<\/p>\n\n\n\n<p><\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"680\" height=\"392\" src=\"https:\/\/transparency.com.al\/wp-content\/uploads\/2023\/11\/laborator-koncsion.jpg\" alt=\"\" class=\"wp-image-2277\" srcset=\"https:\/\/old.transparency.com.al\/wp-content\/uploads\/2023\/11\/laborator-koncsion.jpg 680w, https:\/\/old.transparency.com.al\/wp-content\/uploads\/2023\/11\/laborator-koncsion-300x173.jpg 300w, https:\/\/old.transparency.com.al\/wp-content\/uploads\/2023\/11\/laborator-koncsion-18x10.jpg 18w, https:\/\/old.transparency.com.al\/wp-content\/uploads\/2023\/11\/laborator-koncsion-600x346.jpg 600w\"  sizes=\"(max-width: 680px) 100vw, 680px\" \/><\/figure>\n<\/div>\n\n\n<p><\/p>\n\n\n\n<p>Nga auditimi i Kontrat\u00ebs koncesionare\/PPP \u201cSh\u00ebrbimi laboratorik\u201d ofrimi i k\u00ebtij sh\u00ebrbimi \u00ebsht\u00eb i kufizuar, nga kushtet e kontrat\u00ebs pika 22.4 \u201cTavanet e buxhetit\u201d.<\/p>\n\n\n\n<p>\u201cTavanet e buxhetit\u201d, sipas t\u00eb cilit koncesionari nuk mund t\u00eb tejkaloj\u00eb fondin e miratuar p\u00ebr spitalet n\u00eb z\u00ebrin \u201cLaborator\u00ebt\u201d, p\u00ebr rrjedhoj\u00eb rastet shtes\u00eb duhet t\u00eb transferohen n\u00eb nj\u00eb list\u00eb pritje me p\u00ebrpar\u00ebsi, duke filluar nga periudha tjet\u00ebr buxhetore e spitalit referues.<\/p>\n\n\n\n<p>Nga auditimi \u00ebsht\u00eb evidentuar se 11 nga 19 spitale (QSU, SU \u201cShefqet Ndroqi\u201d, SU \u201cTrauma\u201d, spitalet Elbasan, Fier, Kor\u00e7\u00eb, Sarand\u00eb etj.) kan\u00eb nj\u00eb nivel t\u00eb realizimit t\u00eb sh\u00ebrbimit laboratorik n\u00eb mas\u00ebn 97-100 %, kundrejt planit.<\/p>\n\n\n\n<p>N\u00eb k\u00ebto spitale ofrimi i sh\u00ebrbimit laboratorik ka pasur kufizim respektimin e tavaneve buxhetore dhe ka rrezikuar t\u00eb b\u00ebhet penges\u00eb p\u00ebr ofrimin e analizave laboratorike, n\u00eb mas\u00ebn e duhur, sipas nevojave spitalore, duke sjell\u00eb p\u00ebr pasoj\u00eb drejtimin e pacient\u00ebve drejt\u00eb laborator\u00ebve privat\u00eb, me kosto p\u00ebr qytetar\u00ebt, si dhe p\u00ebrkeq\u00ebsim t\u00eb sh\u00ebndetit nga moskryerja n\u00eb koh\u00eb e analizave p\u00ebrkat\u00ebse.<\/p>\n\n\n\n<p>Ofrimi i k\u00ebtij sh\u00ebrbimi sipas k\u00ebrkes\u00ebs, \u00ebsht\u00eb i kufizuar, nga kushtet e kontrat\u00ebs ku cil\u00ebsohet se, \u201cN\u00eb \u00e7do periudh\u00eb t\u00eb caktuar buxhetore, koncesionari do t\u00eb njoftoj\u00eb drejtorin e spitalit referues, duke v\u00ebn\u00eb n\u00eb dijeni dhe p\u00ebrfaq\u00ebsuesin e FSDKSH, brenda dy dit\u00ebve kur tarifat e sh\u00ebrbimeve t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga koncesionari p\u00ebr \u00e7do spital referues tejkalojn\u00eb 50%; 75% dhe 100%, p\u00ebrkat\u00ebsisht t\u00eb tavanit buxhetor t\u00eb zbatuesh\u00ebm p\u00ebr at\u00eb spital\u201d.<\/p>\n\n\n\n<p>Kur n\u00eb nj\u00eb periudh\u00eb t\u00eb vetme buxhetore, tarifat e sh\u00ebrbimit t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga kocesionari, p\u00ebr \u00e7do spital referues, arrijn\u00eb 75 % t\u00eb tavanit t\u00eb buxhetit dhe koncesionari e ka njoftuar drejtorin e spitali referues n\u00eb p\u00ebrputhje me kontrat\u00ebn. Drejtori i spitalit referues brenda 3 dit\u00ebve, informon me shkrim koncesionarin duke v\u00ebn\u00eb dijeni autoritetin dhe FSDKSH n\u00eb lidhje me vendimin e tij p\u00ebr t\u00eb:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>vazhduar me marrjen e sh\u00ebrbimeve mjek\u00ebsore, p\u00ebr t\u00eb gjith\u00eb pacient\u00ebt e referuar deri sa t\u00eb arrihet 100% i tavanit buxhetor (alternativa A)<\/li>\n\n\n\n<li>vazhduar me marrjen e sh\u00ebrbimeve mjek\u00ebsore vet\u00ebm p\u00ebr analizat urgjente deri sa t\u00eb arrihet 100% i tavanit buxhetor.<\/li>\n<\/ol>\n\n\n\n<p>Koncesionari ka detyrim t\u00eb ofroj\u00eb analiza urgjente, pa pages\u00eb spitalit referues, brenda nj\u00eb v\u00ebllimi t\u00eb lejuar q\u00eb mbulon 5 % t\u00eb v\u00ebllimit t\u00eb analizave urgjente, t\u00eb kryera p\u00ebr spitalin referues, deri n\u00eb p\u00ebrmbushjen e tavanit buxhetor.<\/p>\n\n\n\n<p>Nga sa m\u00eb sip\u00ebr, rezulton se kufizimet n\u00eb kryerjen e analizave laboratorike jan\u00eb t\u00eb lidhura me tavanin buxhetor t\u00eb secilit spital dhe jan\u00eb t\u00eb sanksionuara n\u00eb kontrat\u00ebn koncesionare t\u00eb laborator\u00ebve me pasoja negative n\u00eb sh\u00ebndetin e popullat\u00ebs.<\/p>\n\n\n\n<p>Kontrata e laborator\u00ebve parashikon pages\u00ebn e nivelit t\u00eb sh\u00ebrbimit nga ana e spitaleve publike p\u00ebr nj\u00eb periudh\u00eb 10 vje\u00e7are, ku tavani buxhetor sipas pik\u00ebs 1.1 t\u00eb kontrat\u00ebs n\u00ebnkupton \u201cShum\u00ebn maksimale totale t\u00eb tarif\u00ebs s\u00eb sh\u00ebrbimit t\u00eb pagueshme nga nj\u00eb spital referues p\u00ebr koncesionarin n\u00eb nj\u00eb periudh\u00eb buxhetore gjat\u00eb afatit, e cila p\u00ebrfaq\u00ebson tavanin buxhetor vjetor p\u00ebr nj\u00eb spital referues n\u00eb 12 periudha buxhetore (muaj)\u201d.<\/p>\n\n\n\n<p>Nd\u00ebrsa n\u00eb shtojc\u00ebn 12 t\u00eb kontrat\u00ebs \u00ebsht\u00eb p\u00ebrcaktuar se: \u201cSpitali referues angazhohet p\u00ebr p\u00ebrmbushjen e \u00e7do detyrimi q\u00eb i ngarkohet nga kontrata, por pa kufizim pages\u00ebn e tarifave t\u00eb sh\u00ebrbimit t\u00eb koncesionarit gjat\u00eb afatit t\u00eb kontrat\u00ebs\u201d. Referuar t\u00eb dh\u00ebnave mbi shpenzimet e kryera n\u00eb vitin 2020 dhe 2021, n\u00eb shumat respektive 1,103,199 mij\u00eb lek\u00eb dhe 1,612,914 mij\u00eb lek\u00eb, si dhe ecuris\u00eb s\u00eb planifikimit afatmes\u00ebm t\u00eb buxhetit p\u00ebr financimin e k\u00ebtij sh\u00ebrbimi rezulton se, vlera e tij do shkoj\u00eb m\u00eb tep\u00ebr se parashikimi, duke k\u00ebrkuar \u00e7do vit m\u00eb shum\u00eb angazhim fondesh buxhetore, analizon KLSH-ja.<\/p>\n\n\n\n<p>KLSH shprehet se edhe p\u00ebrcaktimi i tavaneve vjetore buxhetore, p\u00ebr disiplinimin e shpenzimeve, nuk ka arritur t\u00eb jet\u00eb n\u00eb nivelin e parashikuar, shtuar k\u00ebtu dhe faktin q\u00eb ky kufizim ka t\u00eb b\u00ebj\u00eb drejtp\u00ebrs\u00ebdrejti me plot\u00ebsimin e nevojave t\u00eb pacient\u00ebve n\u00eb koh\u00ebn e duhur.<\/p>\n\n\n\n<p>Gjat\u00eb vitit 2022, plani fillestar p\u00ebr PPP-Laborator\u00eb \u00ebsht\u00eb n\u00eb shum\u00ebn 1,574, mij\u00eb lek\u00eb, si dhe \u00ebsht\u00eb shtuar gjat\u00eb vitit Plani p\u00ebrfundimtar rezulton n\u00eb shum\u00ebn 1,639 mij\u00eb lek\u00eb p\u00ebr 2022.<\/p>\n\n\n\n<p>Koncesioni laboratorik i spitaleve universitare, rajonale dhe atyre bashkiake t\u00eb Sarand\u00ebs dhe Lushnj\u00ebs u firmos n\u00eb mbledhjen e 24 prillit 2019 nga qeveria dhe hyri n\u00eb fuqi n\u00eb dhjetor t\u00eb vitit 2019.<\/p>\n\n\n\n<p>Vlera e kontrat\u00ebs \u00ebsht\u00eb 13 miliard\u00eb lek\u00eb, ose rreth 106 milion\u00eb euro, dhe afati i saj p\u00ebrfundon n\u00eb vitin 2028. Por pagesat p\u00ebr k\u00ebt\u00eb koncesion jan\u00eb rritur vit pas viti. Nga 1.1 miliard\u00eb lek\u00eb q\u00eb u paguan nga buxheti n\u00eb 2021, shuma u rrit n\u00eb 1.6 miliard\u00eb lek\u00eb n\u00eb 2022 dhe pritet t\u00eb arrij\u00eb n\u00eb 1.8 miliard\u00eb lek\u00eb n\u00eb 2023, sipas t\u00eb dh\u00ebnave t\u00eb Ministris\u00eb s\u00eb Financave.<\/p>\n\n\n\n<p>Nd\u00ebrsa n\u00eb 2024-n n\u00eb projektbuxhet jan\u00eb parashikuar 1.85 miliard\u00eb lek\u00eb. Ofrimi i k\u00ebtij sh\u00ebrbimi sipas k\u00ebrkes\u00ebs, \u00ebsht\u00eb i kufizuar, nga kushtet e kontrat\u00ebs ku cil\u00ebsohet se, \u201cN\u00eb \u00e7do periudh\u00eb t\u00eb caktuar buxhetore, koncesionari do t\u00eb njoftoj\u00eb drejtorin e spitalit referues, duke v\u00ebn\u00eb n\u00eb dijeni dhe p\u00ebrfaq\u00ebsuesin e FSDKSH, brenda dy dit\u00ebve kur tarifat e sh\u00ebrbimeve t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga koncesionari p\u00ebr \u00e7do spital referues tejkalojn\u00eb 50%; 75% dhe 100%, p\u00ebrkat\u00ebsisht t\u00eb tavanit buxhetor t\u00eb zbatuesh\u00ebm p\u00ebr at\u00eb spital\u201d. Kur n\u00eb nj\u00eb periudh\u00eb t\u00eb vetme buxhetore, tarifat e sh\u00ebrbimit t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga kocesionari, p\u00ebr \u00e7do spital referues, arrijn\u00eb 75 % t\u00eb tavanit t\u00eb buxhetit dhe koncesionari e ka njoftuar drejtorin e spitali referues n\u00eb p\u00ebrputhje me kontrat\u00ebn. Drejtori i spitalit referues brenda 3 dit\u00ebve, informon me shkrim koncesionarin duke v\u00ebn\u00eb dijeni autoritetin dhe FSDKSH n\u00eb lidhje me vendimin e tij p\u00ebr t\u00eb: Koncesionari ka detyrim t\u00eb ofroj\u00eb analiza urgjente, pa pages\u00eb spitalit referues, brenda nj\u00eb v\u00ebllimi t\u00eb lejuar q\u00eb mbulon 5 % t\u00eb v\u00ebllimit t\u00eb analizave urgjente, t\u00eb kryera p\u00ebr spitalin referues, deri n\u00eb p\u00ebrmbushjen e tavanit buxhetor. Nga sa m\u00eb sip\u00ebr, rezulton se kufizimet n\u00eb kryerjen e analizave laboratorike jan\u00eb t\u00eb lidhura me tavanin buxhetor t\u00eb secilit spital dhe jan\u00eb t\u00eb sanksionuara n\u00eb kontrat\u00ebn koncesionare t\u00eb laborator\u00ebve me pasoja negative n\u00eb sh\u00ebndetin e popullat\u00ebs. Nga auditimi \u00ebsht\u00eb evidentuar se 11 nga 19 spitale (QSU, SU \u201cShefqet Ndroqi\u201d, SU \u201cTrauma\u201d, spitalet Elbasan, Fier, Kor\u00e7\u00eb, Sarand\u00eb etj.) kan\u00eb nj\u00eb nivel t\u00eb realizimit t\u00eb sh\u00ebrbimit laboratorik n\u00eb mas\u00ebn 97-100 %, kundrejt planit. N\u00eb k\u00ebto spitale ofrimi i sh\u00ebrbimit laboratorik ka pasur kufizim respektimin e tavaneve buxhetore dhe ka rrezikuar t\u00eb b\u00ebhet penges\u00eb p\u00ebr ofrimin e analizave laboratorike, n\u00eb mas\u00ebn e duhur, sipas nevojave spitalore, duke sjell\u00eb p\u00ebr pasoj\u00eb drejtimin e pacient\u00ebve drejt\u00eb laborator\u00ebve privat\u00eb, me kosto p\u00ebr qytetar\u00ebt, si dhe p\u00ebrkeq\u00ebsim t\u00eb sh\u00ebndetit nga moskryerja n\u00eb koh\u00eb e analizave p\u00ebrkat\u00ebse. Ofrimi i k\u00ebtij sh\u00ebrbimi sipas k\u00ebrkes\u00ebs, \u00ebsht\u00eb i kufizuar, nga kushtet e kontrat\u00ebs ku cil\u00ebsohet se, \u201cN\u00eb \u00e7do periudh\u00eb t\u00eb caktuar buxhetore, koncesionari do t\u00eb njoftoj\u00eb drejtorin e spitalit referues, duke v\u00ebn\u00eb n\u00eb dijeni dhe p\u00ebrfaq\u00ebsuesin e FSDKSH, brenda dy dit\u00ebve kur tarifat e sh\u00ebrbimeve t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga koncesionari p\u00ebr \u00e7do spital referues tejkalojn\u00eb 50%; 75% dhe 100%, p\u00ebrkat\u00ebsisht t\u00eb tavanit buxhetor t\u00eb zbatuesh\u00ebm p\u00ebr at\u00eb spital\u201d. Kur n\u00eb nj\u00eb periudh\u00eb t\u00eb vetme buxhetore, tarifat e sh\u00ebrbimit t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga kocesionari, p\u00ebr \u00e7do spital referues, arrijn\u00eb 75 % t\u00eb tavanit t\u00eb buxhetit dhe koncesionari e ka njoftuar drejtorin e spitali referues n\u00eb p\u00ebrputhje me kontrat\u00ebn. Drejtori i spitalit referues brenda 3 dit\u00ebve, informon me shkrim koncesionarin duke v\u00ebn\u00eb dijeni autoritetin dhe FSDKSH n\u00eb lidhje me vendimin e tij p\u00ebr t\u00eb: Koncesionari ka detyrim t\u00eb ofroj\u00eb analiza urgjente, pa pages\u00eb spitalit referues, brenda nj\u00eb v\u00ebllimi t\u00eb lejuar q\u00eb mbulon 5 % t\u00eb v\u00ebllimit t\u00eb analizave urgjente, t\u00eb kryera p\u00ebr spitalin referues, deri n\u00eb p\u00ebrmbushjen e tavanit buxhetor. Ofrimi i k\u00ebtij sh\u00ebrbimi sipas k\u00ebrkes\u00ebs, \u00ebsht\u00eb i kufizuar, nga kushtet e kontrat\u00ebs ku cil\u00ebsohet se, \u201cN\u00eb \u00e7do periudh\u00eb t\u00eb caktuar buxhetore, koncesionari do t\u00eb njoftoj\u00eb drejtorin e spitalit referues, duke v\u00ebn\u00eb n\u00eb dijeni dhe p\u00ebrfaq\u00ebsuesin e FSDKSH, brenda dy dit\u00ebve kur tarifat e sh\u00ebrbimeve t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga koncesionari p\u00ebr \u00e7do spital referues tejkalojn\u00eb 50%; 75% dhe 100%, p\u00ebrkat\u00ebsisht t\u00eb tavanit buxhetor t\u00eb zbatuesh\u00ebm p\u00ebr at\u00eb spital\u201d. Kur n\u00eb nj\u00eb periudh\u00eb t\u00eb vetme buxhetore, tarifat e sh\u00ebrbimit t\u00eb pagueshme p\u00ebr numrin e p\u00ebrgjithsh\u00ebm t\u00eb analizave t\u00eb kryera nga kocesionari, p\u00ebr \u00e7do spital referues, arrijn\u00eb 75 % t\u00eb tavanit t\u00eb buxhetit dhe koncesionari e ka njoftuar drejtorin e spitali referues n\u00eb p\u00ebrputhje me kontrat\u00ebn.
