Objektivi i k\u00ebtij koncesioni ishte p\u00ebrmir\u00ebsimi i cil\u00ebsis\u00eb dhe siguris\u00eb s\u00eb sh\u00ebrbimeve kirurgjikale n\u00eb spitalet publike n\u00eb t\u00eb gjith\u00eb vendin. Por me kalimin e viteve, ky projekt u kthye nga nj\u00eb premtim modernizimi n\u00eb nj\u00eb shembull flagrant t\u00eb sfidave q\u00eb shoq\u00ebrojn\u00eb kontratat koncesionare n\u00eb Shqip\u00ebri. Nd\u00ebrsa ideja e financimit \u201cpay-per-use\u201d, ku shteti paguan p\u00ebr \u00e7do p\u00ebrdorim t\u00eb sh\u00ebrbimit, synonte t\u00eb sillte eficienc\u00eb dhe kontroll t\u00eb kostove, realiteti rezultoi krejt ndryshe. Nga viti 2017 deri n\u00eb 2021, pagesat ndaj kompanis\u00eb kap\u00ebn shifr\u00ebn e 7.88 miliard\u00eb lek\u00ebve, nd\u00ebrsa n\u00eb total, buxheti parashikon t\u00eb paguaj\u00eb deri n\u00eb fund t\u00eb kontrat\u00ebs rreth 16.5 miliard\u00eb lek\u00eb. N\u00eb m\u00ebnyr\u00eb t\u00eb befasishme, vet\u00ebm p\u00ebr vitin 2018, shpenzimet tejkaluan ndjesh\u00ebm parashikimet fillestare: nga 1.26 miliard\u00eb lek\u00eb t\u00eb planifikuara, u faturuan rreth 2.08 miliard\u00eb lek\u00eb. Nj\u00eb rritje e till\u00eb nuk u shoq\u00ebrua me asnj\u00eb analiz\u00eb t\u00eb thelluar apo nd\u00ebrhyrje p\u00ebr t\u00eb mbajtur kostot n\u00ebn kontroll. P\u00ebr m\u00eb tep\u00ebr, strukturat p\u00ebrgjegj\u00ebse nuk kishin parashikuar mekanizma p\u00ebr monitorim dhe kontroll t\u00eb zbatimit t\u00eb kontrat\u00ebs n\u00eb terren. N\u00eb auditimet e kryera nga Kontrolli i Lart\u00eb i Shtetit (KLSH), evidentohet se mungon nj\u00eb nj\u00ebsi funksionale p\u00ebr kontrollin fizik t\u00eb k\u00ebtij sh\u00ebrbimi, nj\u00eb neglizhenc\u00eb serioze p\u00ebr nj\u00eb kontrat\u00eb q\u00eb r\u00ebndon ndjesh\u00ebm mbi financat publike. Standarde jo t\u00eb barabarta, sete t\u00eb pap\u00ebrshtatshme Problemet e koncesionit t\u00eb sterilizimit nuk kufizohen vet\u00ebm n\u00eb tejkalimet buxhetore dhe munges\u00ebn e monitorimit institucional. Nj\u00eb nd\u00ebr \u00e7\u00ebshtjet m\u00eb shqet\u00ebsuese q\u00eb kan\u00eb dal\u00eb nga auditimet e Kontrollit t\u00eb Lart\u00eb t\u00eb Shtetit (KLSH) ka t\u00eb b\u00ebj\u00eb me munges\u00ebn e standardizimit dhe koherenc\u00ebs n\u00eb ofrimin e seteve kirurgjikale. T\u00eb dh\u00ebnat e vitit 2022 tregojn\u00eb nj\u00eb praktik\u00eb t\u00eb paqart\u00eb dhe t\u00eb pasistemuar n\u00eb shp\u00ebrndarjen e seteve kirurgjikale n\u00eb spitale t\u00eb ndryshme. P\u00ebr t\u00eb nj\u00ebjt\u00ebn nd\u00ebrhyrje kirurgjikale, pra p\u00ebr t\u00eb nj\u00ebjtin lloj operacioni me t\u00eb nj\u00ebjtat k\u00ebrkesa klinike,&nbsp; jan\u00eb p\u00ebrdorur sete t\u00eb ndryshme instrumentesh. Kjo jo vet\u00ebm q\u00eb cenon standardet klinike dhe sigurin\u00eb e pacientit, por krijon edhe nj\u00eb konfuzion t\u00eb madh n\u00eb logjistik\u00ebn e ofrimit t\u00eb sh\u00ebrbimit dhe n\u00eb menaxhimin e kostove. N\u00eb praktik\u00eb, mungesa e nj\u00eb protokolli t\u00eb unifikuar p\u00ebr p\u00ebrcaktimin e p\u00ebrmbajtjes s\u00eb seteve t\u00eb personalizuara sjell pasoja t\u00eb shumta. Nga nj\u00ebra an\u00eb, stafi kirurgjikal nuk ka garanci p\u00ebr pajisje t\u00eb nj\u00ebjta dhe t\u00eb p\u00ebrshtatshme nga nj\u00eb nd\u00ebrhyrje n\u00eb tjetr\u00ebn. Kjo mund t\u00eb ndikoj\u00eb drejtp\u00ebrdrejt n\u00eb koh\u00ebzgjatjen e nd\u00ebrhyrjes, n\u00eb p\u00ebrgatitjen para operacionit dhe potencialisht n\u00eb sigurin\u00eb intra-operatore t\u00eb pacientit.
